Dear Prospective Parents/Carers,
When filling out this enrolment form please pay particular attention to the Parent Occupation Codes as this reflects on the amount of funding our school acquires, the higher the code the less funding we are entitled to.
As well as completing the attached enrolment form it is a requirement of the Department of Education and Early Childhood Development that you also supply a copy of the child’s birth certificate and immunisation certificate.
If you have misplaced your child’s birth certificate a copy can be obtained from the Registry of Births, Deaths & Marriages, Victoria. Phone: 1300 369 367.
School entry immunisation status certificates can be obtained from the Australian Childhood Immunisation Register (ACIR), they can be contacted on 1800 653 809 or accessed through the myGov website.
In the event that your child is not immunised and you are a Conscientious Objector a letter from your medical practitioner has to be supplied.
